Sample Acceptance Criteria
Blood sample
- Properly filled Requisition forms.
- Properly labeled samples.
- Details on Requisition form should be matched with sample container details
- Proper Vacutainers
- Appropriate quantity of sample.
- ABG sample & Plasma sample should not be clotted
- Spill over the sample should not be there.
- Sample will be received within 4 hours of collection.
Urine Sample:
- Properly filled Requisition forms.
- Properly labeled samples.
- Details on Requisition form should be matched with sample container details
- Appropriate quantity of sample.
- Sample containers should not be open, they should be tightly packed.
- Sample spillage should not be there.
All specimens must have the following information on the vacutainer:
- Patient’s Name
- Ward Name
- Specific Case Registration Number (MRD Number)
- Date
Information present in requisition form must match with information present on vacutainer.
For ABG analysis specimens must be received on ice packs in pre-heparinized syringe and as early as possible.
Criteria For Sample Rejection:
Blood sample:
- Improperly filled Requisition forms.
- Improperly labeled samples.
- Details on Requisition form is not matched with sample container details
- Improper Vacutainers
- Insufficient sample volume / Overfilled sample
- Clotted sample (Whole blood/Plasma)
- Spill over sample.
- Hemolyzed sample.
- Sample received after 4 hours of collection.
- Diluted Sample
Urine Sample:
- Improperly filled Requisition forms.
- Improperly labeled samples.
- Details on Requisition form is not matched with sample container details
- Insufficient sample volume.
- Improper sample container
- Spill over sample.
If prepared serum or plasma is grossly hemolyzed.
Sample is broken or has leaked in transit.
Sample acceptance Exceptions
A.Insufficient sample volume.
- Unordered List ItemDo not reject the sample.
- Unordered List ItemPerform as many examinations as possible as per priority.
- Unordered List Item Priority of testing will be decided by the laboratory incharge or consultant.
- Unordered List ItemInform the customer.
- Unordered List ItemEnter remarks of insufficient volume in the report.
- Unordered List ItemAll pediatric samples with insufficient volume will be received by the laboratory.
