**Purpose and objectives:**
The purpose of this Policy is to ensure that verbal requests made by physicians are correctly managed. 

**Responsibilities:**
Laboratory Technician & Section In charge

**Procedure:**

  * In the event that the clinician requesting investigations has already sent the request form and sample/s to the laboratory and then decides to add another test/s, the laboratory allows it to be done. 

**The procedure is as follows:** 
  - The physician contacts the laboratory to request additional testing be performed on the previously received sample
  - The laboratory staff receiving the call verifies that the appropriate sample was received, there is sufficient volume to do the further tests, and that the sample's integrity is still appropriate for the testing;
  - The laboratory staff confirms to the requesting clinician that the additional tests will be done.
  - The laboratory staff informs the requesting clinician to fill-in another request form for the additional tests; in the meantime, the sample will be processed to ensure the results are not delayed.
  - Laboratory enters the order in the LIS. This allows the new study to be assigned the same accession number as the specimen already in the laboratory.
  - Results of the additional tests will only be released upon receiving the request form.
  - If any of the above conditions are not met, then another sample should be collected.
