SOP of Workflow of clinical pathology (outdoor section)

  • First of all the patient comes at registration window with case paper & request form with desired investigations and registered at the registration desk.
  • Registration includes name, age, sex, referred OPD, MRD number, investigations desired along with the unique lab ID number. This unique ID number is noted on patient’s main hospital registration form for future reference.
  • Simultaneously patient is registered on HMIS (computer)
  • Then patient is guided towards next window of RogiKalyanSamiti. Patient is required to deposit Rs.25/- here. Some patients are exempted from this fee according to the set protocols.
  • Patients are then given labeled container according to the desired investigations written on the request form.
  • Patients are then guided towards the entrance door of laboratory for the waiting area.
  • Patients are then guided for collection of prescribed investigation i.e urine.
  • After collection, sample is put by patient in clinical pathology cabin. Then immediately sample is processed by technician.
  • On receiving the samples in clinical pathology processing room, technician will first perform physical examination of urine and then after chemical examination is performed with two/three parameter (protein, glucose and pH).
  • If additional specific chemical tests (Bilirubin, specific gravity, blood) are requested by the clinician, it is performed by using multistix urine strips or by automated analyzer. If multistix urine strips are not available then urine samples are checked with two/three parameter urine strips along with manual method.
  • Results of physical and chemical examination are entered in report forms and in the register by technician
  • After chemical examination, urine is centrifuged at 2500 rpm for 5min, supernatant is transferred in another container with labeling of sample ID.
  • Test tubes with sediment are given to doctors with their request form & report forms in clinical pathology reporting room for microscopic examination.
  • First, Second and third year resident doctor perform microscopic examination routinely. All positive urine samples and random five negative samples in a day are also counterchecked by senior resident or by tutor in the section.
  • The results are noted in the report forms itself and then sent for report entry in reporting software.
  • When patient comes and shows the acknowledgement slip on which unique ID lab number is written the reports are searched for, and on printing respective reports are given to the patient and the lab number that is written on his/her main form is struck through to indicate that the report has been dispatched to the patient.
