Type and amount of sample to be collected==== Enough biopsy material or tissue specimen should be collected for histopathological examination. ====3. Special timing of collection==== Not applicable ====4. Instructions to clinicians for selection of examinations==== -Submission of request form properly filled with pt’s name,age sex ,reg no.,ward.,relevant clinical history,past history,previous history or FNAC reports if any with the source of material is important -Surgical specimen and biopsy should be properly labelled for identification. ====5. Preparation of the patient by pathologist.==== Not applicable ====6. Procedure of primary sample collection==== Surgical biopsy and surgical specimens are received at Histopathology section with complete clinical details and properly filled request. ====7. Identification of primary sample==== Tissue submitted for histopathological examination should be preserved in 10% formalin, should be properly labelled with the name, reg. no, ward, age, sex, clinical diagnosis and with properly filled request form. In case of frozen section tissue should be send in Normal saline. ====8. Relevant Clinical information==== Mention following clinical information in request form: *Complete clinical history *Probable clinical diagnosis *Any relevant previous history *Any investigation done for the same(blood,radiological findings etc.) *Any treatment taken for the same *FNAC report if available ====9.
