=====SOP of Workflow of clinical pathology(indoor section)===== *First of all the samples and request forms are received at the reception counter. They are checked for appropriate information and patient details. Provided request form is matched for patient details and other desired data. If sample is rejected according to the written criteria it is mentioned in rejection register along with reason for rejection and action taken. *Samples are registered in sample receiving register at the reception desk and are given a lab ID no. * Registration includes name, age, sex, ward, MRD number, investigations desired along with the unique lab ID number. The request form is provided with a counter slip attached to its lower part. This is filled and given back to the servant or the patient relative, who brought the sample to the lab, for further reference. * Then samples are aligned sequentially according to the specific investigations desired. * The labeled filled container with the respective request forms are then transferred to the processing room within appropriate time. * On receiving the samples in clinical pathology processing room, technician will first perform physical examination of urine and then after chemical examination is performed with two/three parameter (protein, glucose and pH). * If additional specific chemical tests (Bilirubin, specific gravity, blood) are requested by the clinician, it is performed by using multistix urine strips or by automated analyzer. If multistix urine strips are not available then urine samples are checked with two/three parameter urine strips along with manual method. * Results of physical and chemical examination are entered in report forms and in the register by technician * After chemical examination urine is centrifuged at 2500 rpm for 5 mins, supernatant is transferred in another container with labeling of sample ID. * Test tubes with sediment are given to doctors with their request form & report forms in clinical pathology reporting room for microscopic examination. * First, second and third year resident doctor performs microscopic examination routinely. All positive urine samples and random five negative samples in a day are also counterchecked by senior resident or by tutor in the section. * The results are noted in the report forms itself and then sent for report entry in reporting software. * Finally the reports are aligned ID number wise and are dispatched to the respective wards after being signed by the authorized signatory. * Records of dispatched reports are written on the respective request forms.
